How to complete your intake form

After you purchase or book, an intake form link is emailed to you. Sign in to the member portal, answer the questions, and your practitioner reads it before your session.

Complete your intake form as soon as you can after purchasing or booking: the link arrives by email, and your practitioner reads your answers before creating your session. Your answers directly shape what happens in that session, so this is the single most useful thing you can do before we meet.

Prerequisites

You need the email address you used at purchase or booking, since the form is linked to your account.

Steps

  1. After your purchase or booking, look for the email from Norwest Wellbeing containing your intake form link.
  2. Click the link, or sign in to the member portal at members.norwestwellbeing.com.au: your form is waiting there too. If you haven't signed in before, see how to sign in to the member portal.
  3. Answer the questions honestly and in your own words. There are no wrong answers. Plain descriptions of what's happening and what you want to change are exactly what your practitioner needs.
  4. Submit the form. Your practitioner reads it before creating your session, whether that's a live appointment or a personalised audio recording.

Which form you see depends on your situation: new clients complete the full intake, returning clients get a short check-in, and child or teen clients have their own versions. intake forms explained covers the differences.

Troubleshooting

If you can't find the form email, check your spam folder first, then sign in to the portal directly: the form is available there without the email. If you booked for your child, you complete the form on their behalf; for teenagers, see teen intake forms. Anything else, email hello@norwestwellbeing.com.au.
